How are financial arrangements handled?

The facility charge incorporates all care during your procedure but does NOT include pre-procedure tests, anesthesia professional fee, pathology studies or special supplies.

All co-pays, deductibles and co-insurance are due at the time of admission as determined by your insurance benefits pertaining to ambulatory surgery centers. Cosmetic procedures and self-pay accounts require full payment (100%) at the time of admission. If special payment arrangements are necessary, please call the Business Office prior to the day of surgery. Our Central Billing Office can be reached Monday through Friday between the hours of 8:00 AM and 5:00 PM at (904) 737-9818 for billing questions.

Cash, Checks, Money Orders, MasterCard, Visa, American Express and Discover are accepted forms of payment.
