IF YOU HAVE POWER OF ATTORNEY OR GUARDIANSHIP of the patient, please bring legal documentation of such. THERE WILL BE NUMEROUS FORMS to sign and to be filled out that pertain to your state of health and personal information.

TO PROTECT YOUR PRIVACY the only people that will be allowed in the treatment area are the ones you list on the consent form - which you will fill out in the registration area. Only two people will be allowed in the treatment area at a time.
